• Modifications for ICTS resource requests to already approved protocols. Please include the prior IRB number in the title.

This application process is divided into several sections. Please note:

• You may cancel changes any time and revert to the last saved state by clicking Cancel.
• You can log into the application website at any time to complete an unfinished protocol application.

• E-IRB application (required)
• Consent (if required, up to 4 files)

• Master protocol for Industry sponsored or Multi-center trials
